 Provider Portal Registration

To create your account, please fill out this form and then create your login on our  [Provider Portal](https://d2yhnf04.na1.hs-sales-engage.com/Ctc/GG+23284/d2yHNf04/JkM2-6qcW6N1vHY6lZ3kMN5RrPngr0XsfW7kjXv73Q2vwNVWm-dt85ytXFW8QykCs8K2mr8W2LY1yq6Vt37KW32PBK97dxP22VTsgr88j4hnGW7Gnn2n7kKP80W2x0j4z7PH2MnN2VnzXPc86wyW5R_WqT6FYgYgW8Hn3l91v_wwYW194lBV8mqSjvV_TNxl8D99LwN2-BWgy2QpGCW1lJ_Gt35p_nnVTrnRm1_zC8hW4t4s9b8284kHW53xK_T7tzjmQW5NMz2c6jlf-GW30LGD-89Yy3-W88Bjg96HmRjsf2ScrXq04). You can access a Set Up Guide or watch this instructional walkthrough [video](https://storage.googleapis.com/gmd-public/portal_walkthrough.mp4) of the registration and ordering process.

Once your account is verified, you can place patient orders. Upon registration, we will send all providers a welcome email, including a link to our comprehensive onboarding guide. This document gives you detailed information about our test, the science, and all of the logistical pieces of the process.

As a reminder, we are currently only offering a ship-to-home saliva kit, which is not sent until we receive payment from the patient for the test. Please reach out to [support@genomicmd.com](mailto:support@genomicmd.com) at any time with any questions you have. We are here to help and are so excited to work with you!
